Before me, the undersigned authority, personally appeared Norman Linton who duly sworn, depose and says:
1. That he is the Landlord of rented premises located in Hillsborough County, Florida, described as follows:
XXX9 E Mohawk Ave #C, Tampa FL 33604
2. That Defendant(s), Diane Carol XXXX rented the subject premises from the Landlord agreeing to pay the rent of $420.00 per month.
3. The Defendant(s) failed to pay the rent due on the October 2, 1996, whereupon the Landlord, on October 5, 1996, served a three (3) day notice demanding payment of the rent or possession of the premises.
4. The Defendant(s) failed to comply with either of the demands of the notice within the applicable time period.
5. The Defendant(s) owe(s) the Landlord $420.00 for past due rents as at the date of this affidavit.
6. The Defendant(s) is/are not now nor has/have been in the military service of the United States of America since the institution of this action.

THIS CAUSE came on to be heard before the Court upon the Plaintiff's COMPLAINT FOR TENANT EVICTION AND PAST DUE RENT, and it is appearing that the Defendant(s) was/were duly served and said Defendant(s) having failed to file any pleadings contesting the allegations of said COMPLAINT and a DEFAULT having been duly entered, and the Court having taken testimony / or received affidavits from the Plaintiff/Landlord, and being otherwise fully advised in the premises herein, it is
